I believe what these people wish to say, but aren't capable of saying, is yes, someone who enjoys fictional works depicting taboo sexual acts does indeed have a fetish for the taboo sexual act depicted, the fictional nature of the subject matter doesn't not remove the reality of the person's desire to seek out the sexual act in some form or another.
However, consumption of fictional works displaying a taboo act des not mean the consumer will perform the taboo act in real life when given the opportunity.
In many cases I believe you could argue consumption of fictional works depicting taboo behaviours help control the urges people have and as a result they're less likely to act such behaviours out in the real world.
