so does anyone know how drugs work actually?

I mean if the difference between serotonin and dmt is primarily a single carbon atom pair then I would imagine it have something to do with a corresponding ligand in the post-synaptic cleft?

I think I remember reading that no one really knows what the agent of neurotransmitter reuptake is. does that mean no one knows how drugs work?

eh, we know maybe 90% of how drugs work, and that exactly specific mechanism of action is the last 10% we don't know yet.

The true scale and function of serotonin is definitely a topic thats lacking in research, and iirc it depends on the individual what hormones regulate each other. Im not sure if its really down on the ligand level but drugs like DMT have been studied and found to interact with certain variations of 5-HT that don't react as strongly to normal serotonin. So perhaps it is a ligand thing but why have these structures that are rarely used? Did humans evolve to take drugs or is "serotonin" not what we think it is and perhaps the different receptors are more different from each other than we initially assume.

Depends on the class of drug and on the drug specifically; aspirin for example belongs to the group of COX-1 inhibitors, inhibiting the enzyme (cyclooxygenase) that produces prostaglandins and making your head hurt after having a drink too much. On the other hand, though not a drug in a medicinal sense, we have methylmercury and mercury: the latter is rather harmless while the former kills you within a few milligrams.

stop talking bs or elaborate what you mean by that. There is for example an enzyme, let's take the dopamine transporter (DAT) which is located on the cell surface transporting dopamine back into the cytosol. There is no "agent" other than dopamine and sodium (extracellular -> intracellular).. though there might be endogenous modulators that affect (in this case) "transport" enzymes, that however is speculation.
