It starts off establishing that Cliff, Rita, and Larry pretty much have PTSD and might want to die, but they're able to keep themselves together because they have each other.
The Chief is pretty much evil in the series, like unapologetically evil. He readily exploits anything he can for his own safety.
The types of stuff the Doom Patrol deal with in Giffen's run are still the same level of crazy we've come to expect from it. First they deal with a bugmonster lady and her monster bug factory, then there's the sentient black hole that's capable of mass possession.
The black hole was curious about humans, but unfortunately it ended up killing anyone it possessed since once it was forced out of them they'd disintegrate. He did this to like 100,000 people and one of them ended up being a balding slightly out of shape man from a remote German village who had latent psychic powers. The Chief forced Mento to assume direct control of Rita remotely to destroy all the possessed people save for that one guy who became the black hole entity's main representation from then on.
Before we could get into the consequences of it killing like 100,000 people, the Blackest Night event happened and the Doom Patrol were assaulted by Cliff's original body, Negative Woman, the Chief's wife (the lady who founded the second iteration of the team before the Invasion event), and some other guy from a past team.
Rita didn't get to confront Mento over him messing with her mind because of that and the team gets portaled off to the war of light.
Oh, and the Cheif's legs get turned to ash by his zombie bride before black hole man intervenes to save him. This is where Niles begins to fucking lose it, his sense of security is totally compromised and he feels he can no longer trust his team to keep him safe.
