There is no "entire chain". The systems of nuclear launch are designed to be quick, mostly so that they can get missiles in the air for a retaliatory strike before enemy missiles arrive. But that also means that if the president and the SecDef (who, again, is appointed by the President) decide to nuke shit, nobody's in a position to say no.
