Namely that Tony was showing so much worry about people pulling a weapon on him and then that woman hits him with the fact that his actions murdered her son. He created Ultron and kickstarted the loss of all those lives, but he doesn't want anyone to kill him. It wasn't his intention, it wasn't something he had control over. The Hulk, who Banner barely controls at the best of times, has created collateral damage and apparently killed innocent people. He certainly fucked up that city in Age of Ultron. He wasn't in control and used as a weapon, thanks to, of all people, the Scarlet Witch, one of his teammates.
These are the actions that Tony says must be used to keep them in check, under control (but a lot can be said about him just wanting someone else to make the call so they take the blame when shit goes bad. He can say, "It wasn't my fault, I didn't kill your son, it was 117 countries!")
Once it's personal, though, once it's someone he cares about deeply he immediately goes off the rails.
