Tony and Cap are men's men; forgiveness comes hard, but when it does, you're going to be disappointed if you're expecting any overt gestures. Cap's letter to Tony pretty much covers all those bases, anyway, and Tony finally giving Cap back the shield after five years and mentioning how resentment is something he wants to give up is basically his way of asking for Steve's forgiveness for not forgiving him when he should have. It's probably one of the most realistic resolution to a fued between men I've witnessed onscreen.

The intro of Age of Ultron is the Avengers taking down their last stronghold. Any Hydra activity after that is explicitly illicit, and to reinsert anyone back into official positions of power would require recruitment, and lets face it; nobody's lining up to join Hydra anytime soon, if ever again.
Steve's distrust of the governments had more to do with the fact that this was allowed to happen in the first place, and the fact that even politicians not aligned with Hydra were still subject to agendas that might conflict with their efforts to fight threats to public safety, not that he still suspected that HYDRA was still stomping around in the UN or whatever.
