All official lore and tie-in media leading up to the release of Revenge Of The Sith stated he’d been injured in an “accident” and rebuilt by the Separatists, and was very salty about anyone mistaking him for a common droid. Then later in the Clone Wars series, the episode “Lair Of Grievous” suggested that he “chose” augmentations, and that his body was gradually altered one piece at a time. Since this idea was never elaborated on or reintroduced later on, many who prefer the first origin have elected to ignore the claims as Grievous lying to save face, but that’s purely speculation.
