But there's obviously characters like Joker, Harley, Ivy, Black Mask who are clearly supposed to be insane in-canon.

Apart from his sick Batman obsessions and occasional sexual hang-ups, he acts perfectly sane.
He's evil to the core and definitely unhinged but you'd be hard pressed to find a Batman villain who manages to hide his insanity and integrate better into society than him (even Penguin has more hang-ups and difficulties and he's not even insane).

They never seem to make up their minds as to how she's supposed to act but she's really not insane to the point she can't be judged by moral standards. She never seems to have any issues understanding the wrong she's doing to people and she never seems to really care either.
