On Titan, everyone was a perfect Chad/Stacy type beauty... except Thanos. Whereas everyone was a 10/10 hottie, Thanos was a purple skinned hairless freak who was bigger and stronger than everyone else.
(They never explained WHY Thanos was born like this; however it should be noted that the very first Thanos story explicitly showed Mentor and Starfox were also purple skinned/that everyone on Titan was purple skinned but this was dropped ASAP after his first appearance. Other writers implied that because Titans were descendents from humans that were turned into Eternals and Deviants, Thanos was simply born deformed in terms of Deviant DNA in his parents manifesting itself and that Thanos wasn't euthanized at birth, because his dad was the ruler of Titan and pulled a Tywin Lanister and said even deformed, he's my son and as such worthy to live).
Thanos being a deformed monster made him an outcast and Titan was a worthless pacifist planet that outlawed all violence, even guns. Thanos had to deal with his brother Starfox being a lady's man and all the girls mocked Thanos. Mentor later stated that Thanos tortured/raped/murdered a girl who rejected him when he was a kid, but that no one could prove beyond a shadow of a doubt that Thanos did the crime.
