With a narrow fov or higher focal length the camera has to be further away to get the head in the shot and there will be no noticeable perspective differences between something as close as nose vs eyes vs ears. There is no perceptible foreshortening. There might be if you stick your arm out straight since your hand will be much closer to the camera than your nose.
You're right that it's 5 point perspective.
