Fear mainly comes from a lack of understanding: distorsions, deformities, darkness, the feeling of an invader in the safety of your house, turning on the lights in an empty, dark room... in conclusion, pictures that make you ask questions like "Why is this happening?", "What is he looking at?" "What is this doing here?" are quite effective at creating fear. Making the espectator relate to the picture makes everything more effective, doing this with an upfront monster is pretty hard, but removing human features from a guy is a great way to make someone scarier.
