In theory I prefer holding the pen lightly but I still find myself disregarding that preference and accidentally cramping a bit often enough. especially so when drawing details, I think?
Also I like to change things up at times, holding the pen more in the back or front for a while. - admittedly a bit less so in digital it seems (probably becasue I can zoom in and out? not sure...).
But once or twice during an hour of drawing I have to deliberately regrip since the pens casing instead of its tip is contacting the tablet surface. - I happen to slide my fingers way back sometimes, holding the pen at roughly its center (basically at the last half inch / one cm of the rubberized grip section on the intuos pro 4 pen), which leaves the pen at a shallow enough angle for the tip not to reach the drawing surface.
