You could be sued in civil court but it's unlikely it would come to before the C&D. I also imagine that you could circumvent C&Ds by just switching accounts regularly since knowing your actual identity would require a subpoena or something and getting that shit costs too much to do it at scale, but I wouldn't advise it.

The reality though is that the ezpz way to circumvent the whole thing would just be people changing their work slightly and claiming parody. There was the infamous case of nintendo trying to sue a mario porn parody in the 90s and losing the case because it was parody and thus they resorted to buying the rights to the film so they could bury it. So just make some characters look slightly different, add a porn parody disclaimer, and then you're immune so long as you have enough money for a lawyer.
