The complex psychological shit they touched on with Nurse Bendy (essentially regressing into a little girl whos getting raped over and over against her will) and Ms. Sculptham (Who intentionally got raped by Mr. Creepler by dying her hair black because he wouldn't rape blondes. Got pregnant from it, and aborted it in her apartment with a coat hanger) is genuinely fucked with no gags.
The reason why it works in Moral Orel is because theres no retarded Family Guy-type punchlines that kill the tension.
