I mean, at one point he was an aspect of Death - namely 'Death as the ultimate adversary' - while Death of the Endless and the Black Racer were other aspects, namely 'Death as a mercy' and 'Death as an inevitability'.
Personally I see no problem in this - Death is a universal constant, it covers everything - and they may be said to really be no different, they're just the various ways we, mortals, view the end of all that is, just like how Morpheus has many different 'aspects' that are all the same guy, but Neil Gaiman didn't like it, which seems uncharacteristically childish of him.
Death of the Endless implied to Lex Luthor that Nekron's scheme was ultimately irrelevant, but that doesn't really clear up anything.
My headcanon is that after Death of the Endless went through her first 'day as a mortal' - which is a story in itself - she was split into three aspects, one which is wise and kind, one which is angry and resentful and one which is emotionless, but these might just well be states of mind; after all, we seem like different people ourselves depending on when you talk to us.
On the other hand, Nekron has also been stated to be an entirely different entity, the Lord of the Unliving, not Death, but the caretaker of the realm where the dead who have not yet been judged go (apparently there is a big back-up), while 'Blackest Night' seemed to imply he was even older than Death; the embodiment of the non-existence preceding Creation.
