Madame Medusa has a high enough opinion of herself that she thinks she’s not ugly- she even probably thinks her garish makeup looks good on her. Or at least she chooses to act like she thinks that.
But even if Madame M was aware of her own flawed appearance, it doesn’t matter, because she very likely intentionally put Penny down like that just to demotivate her and discourage her from escaping, fully knowing it was a sore spot, so she’d stick around and they could get the diamond. Possibly she didn’t give a fuck what Penny looked like- she was just being mean as a means to get what she wanted
