No, when you hold the apple it's part of the cars reference frame, but when you throw it, it becomes part of the Earth reference frames.
This concept is called inertial frames, look it up:

OP, the real reason the apple would likely not hit the guy at 100mph is because an apple is not going to maintain it's momentum for any meaningful period of time. The air resistance alone would rob the apple of most of it's speed. There's a reason you don't see many drive-by apple killings going on.

Once you throw the apple it no longer has the energy of the car pushing it forward. It will of course carry momentum but this will quickly dissipate because the energy of the car is no longer pushing it through space and air resistance it working against it.

Try sticking your arm out of a car moving 100 mph. The air resistance is pretty crazy. You would have to throw the apple with enough force to overcome that resistance to have it accelerate past the car, and the apple has a very low density, meaning it slows down very quickly.

The only relevance of the dot product would perhaps be to determine the amount of mechanical work the apple achieves because the dot product is a scalar, which would be the product of the apple's force vector and direction product, multiplied by the cos angle between them, wherein the car's initial direction/force would need to be taken into account in addition to the initial conditions of the apple.

So if I'm driving at 100 MPH, in the driver's seat of a 2002 ford mustang and I throw an apple at a bystander walking at 3 MPH in the opposite direction (towards me) who is 6 feet away from my arm towards the pedestrian. If I can throw an apple at let's guess 40 MPH from a sitting position, what is the velocity in which the apple will hit the pedestrian?

Think of the medium of air inside the car and outside two different mediums. You throw the apple outside of the medium going 100 mph, and it is absorbed into the static or non moving medium. Air resistance results because the apple is pushing against a dense collection of air or air particles, like wading through water.

If you rule up the window going 100mph and stick your hand out, you are experience some hurricane force windows moving in the opposite direction of your ditection of travel.

You would be trying to throw the apple into a 100mph headwind.

By the time the apple leaves your hands, gravity will be pulling on it much more than it will be moving toward where you tried to throw it.
