Whatever underreporting occurs now for people with very mild symptoms is the same underreporting that occurred in 2019 when people were having myocarditis from other viral illnesses, if not lower since people right now have symptom checklists like V-safe and are hypervigilant for symptoms they don't necessarily pay attention to in other circumstances. The incidence observed before COVID which is being used to compare the current rate of reports should then be a good comparison.
