Scientifically speaking, what would cause an otherwise healthy 38 year old man's heart to stop due to two blood clots during his routine workout? Surely there must be some cause.

The blood clots are solids. They broke off, perhaps from some old internal injury the dude had already suffered in his prior, recent history. Its not too uncommon. Under the strain of lifting heavy weights, increased pressure, and pulse can dislodge small particles into the bloodstream.

A number of things. Factor V Leiden. Prothrombin G20210A. Those are really common. APC resistance by congenital mutation (other than FVL). Dysfibrogenemia. Deficiencies in protein C or S. Lupus anticoagulant (which, despite its name, isn't related to lupus and isn't an anticoagulant). Anticardiolipin antibodies. At 38, we might think about myelodysplasia or myeloproliferative disorders. This isn't nearly as weird as you probably think.
