the cover causes particles to bounce around and cause a huge radioactive effect. putting ON the cover was the dangerous part.
so they normally have some shims to keep the cover from closing completely. easy. Except canadian cowboy physicist thinks that's too boring, who wants to take out the shims every time, just prop it up with a screwdriver in your other hand
and obviously the screwdriver slips after doing this a dozen times, causing the lid to shut and energy to immediately flare out, thank fuck the guy was smart enough to fling the lid off or there'd still be a radioactive no-go zone there
