Because the Moon's orbit is slightly tilted relative to the Earth's orbit around the Sun.

It only happens when the moon is in the ecliptic plane which is when the moon is in the ascending or descending node of the lunar orbit.
On astronomical clocks that is when the moon is on the dragon hand.
