Each instance was thought to be isolated and no one was making tallies until they started happening frequently enough that someone thought "Hey, maybe this has been going on all along and we weren't looking for it" which turned out to be true.
The Johnson & Johnson vaccine needs a full review of the data about those who have taken it to see if there are two orders of magnitude more people negatively impacted than initially thought as happened with AZ.
