Is it a wise idea to put a 4000 lb forklift on a mezzanine designed to handle a live load of 125 lbs/ft2

You have to calculate the point loads, line loads on the mezzanine, applied in the worst possible location for bending and shear with a high factor of safety, and don't forget the weight that the forklift could be lifting, and the weight of the guy inside.
Like the guy above said you need 4000 lb / 125 psf = 32 ft2 of contact area which you obviously don't have.
You can ask your local structural engineer to do a calculation for you but otherwise you probably shouldn't do it.

you have to say how big the mezzanine is. Live load calculations are done based off the entire size. ie your live load of 125 means a 3ft by 10ft balcony will be able to support 3700lbs of load as long as its spread out across the entire surface

Yeah since items, in general, are made with a safety factor, it can withstand 6 or 7 times the normal load it might deform tough.

Make that 1.1-1.5x normal load. The OP is not talking about a heavy duty truck powertrain axle.

If that were true at each individual square foot, then yes, it would not hold up and your foot would go through. That's an average for the entire structure, so no, nothing will happen
