If you're going topicals try benzoyl peroxide, but make sure to get a good moisturizer because it's going to dry and burn your skin if you don't. If that's not working, you can try antiobiotic topicals, but they usually don't work for very long and require cycling in and out.
Accutane on the other hand is a real shot at a cure for most people. There are some side effects, most of which are related to severe dryness because it dries you the fuck out everywhere - dry skin, dry lips, nosebleeds (dry nose inside skin), dry eyes, constipation (dried out GI tract) etc. The severity of these symptoms very, and you WILL need a good moisturizer, a good lip moisturizer, and to drink a ton of water or you will suffer immensely. Otherwise, the side effects are mostly overblown, and all of the scary ones like depression, balding, etc. don't actually have any evidence supporting them, and any real side effects subside after you finish your course.
My personal experience? I had severe cystic acne that scarred up my face from age 13 to 24 before I finally nutted up and got on it. First cycle was overall mild and my acne was 100% gone by the fourth month. It changed my life. Unfortunately it gradually returned about 3 years after I finished my treatment, but I swore to myself that I would never let it happen again, so I got back on Accutane. Acne cleared within a month, the side effects were even less severe, and I haven't had much of any acne since.
