You definitely can not. I've worked with patients and old people with dementia and schizophrenia. They act like broken machines without realizing it.

You won't really know what's going on.

I've had drug-induced delirium and I kind of forgot I was on a drug, was doing and saying weird shit without really knowing what I was doing. I imagine it's pretty much like that but that's your whole life, horrible.

It does kind of fluctuate though. My grandmother is sometimes fairly with out (though still comes out with strange stuff like talking about her long dead dad) but then she gets an infection and then is either mute or starts racially abusing black nurses.

I find its a lot like how we act in our dreams. Scary monsters, sudden wars, talking animals, all the other kinds of crazy shit we believe in our dreams seems fine and normal while we're there. But when we wake up its obvious how insane and crazy all that shit was. People with dementia are like half stuck in reality and half stuck in dreamland. They're definitely see and comprehend the real world, but its like they're unable to characterize what is and isn't real.

You put it perfectly. Exactly my experience with that delirium/psychosis I mentioned. (not sure if that's quite the right term but it's similar) Felt like I was dreaming while it was awake, didn't question hearing/seeing/doing ridiculous shit and only realised it after. I guess it doesn't seem like hell if you never snap out of it.

Consciousness as in a sense that they are awake sure they are conscious. But someone who sits in a closet for an hour and then tells me he had coffee with his mom definitely (who died 30 years ago) isn't conscious of what's happening to his and can't make any decisions on his own because he doesn't even know where he is. People with dementia are stuck in their memories, unable to form new memories nor they can make any rational conscious decision.

on the outside it appears that overall cognitive abilities decline, but you cannot know for certain what is going on inside their minds, as it’s a matter of 1st person subjective experience

Honestly if you're going to die later in life, dementia isn't so bad. Is it worse than getting a cancer diagnosis and having to look death in the eyes every day knowing your end is imminent? I would say yes. The anosognosia that comes with dementia is actually kinda comfy in a morbid way.
