Large differences in childrens' IQs

What could cause large differences in IQs of siblings?
>parents: A
fairly average in life
bachelor's degrees, stable jobs
>older child: B
struggled with school all his life, tries a shitty meme degree, drops out, tries another useless one, unsurprisingly finds no jobs, tries yet another one
struggles with working independently
not seeing his physique and not knowing he is ~30, and instead judging him by his mannerisms, thinking, even the way of speaking, you would think he is no older than 12
IQ can't be over 100; possibly under 90

seems to be the polar opposite
overachiever throughout his schooling, STEM PhD, stable income
at least midwit level IQ, but certainly above average

I thought older siblings are supposed to be smarter. But still, the differences are quite significant. Are these differences supposed to ease out late in life (C declining toward B's level and both aiming for A's level)? Could B's level be explained by some events post-birth (birth complications, hitting their head hard, etc.)? Could B have been neglected in favour of C, as they would be the younger one - creating a vicious cycle in which C is more and more encouraged as school goes on, while B's failures build up, feeding into more failure?
