All males in my family have a left-sided MASSIVE varicocele. It doesn't hurt at all, and ironically protects that side from testicular torsion. None have had problems knocking women up. In my case, I've had my wives* pregnant within a month of them coming off of birth control (usually by removing their copper IUD myself). We expect confirmation of child #5 tonight given her symptoms, and I removed the thing about 10 days ago.
I don't know why anyone would go through surgery to remove it unless you're that insecure about your lopsided balls. All balls look weird. Theoretically, in some people it could reduce sperm count, but even if that nut was lopped off completely you should have vastly more than necessary to get the job done ... so why?
(I theorize the commonplace nature of them is possibly an evolutionary development. The V causes that nut to hang lower and stay cooler, while also keeping the other cooler from lack of proximity. While it could possibly cause the V nut to produce slightly less sperm, it could be increasing sperm production on the other side through causal processes. I'd love to see a study done on this, because I've never encountered men as fertile as those in my family: we could practically knock up an entire sorority house just by walking by it.)
