Considering our sanitary conditions, is a plague on scale of the Black Death even possible? Like bio weapons, am I crazy for doubting a doomsday scenario

Just look at the modern India or Africa. They are horribly overpopulated, yet all the diseases don't make even a small dip on their population growth. Ok, let's say that you have modern medicine now, but did they have any population-halving epidemics in the last 100-200 years? I bet this was also the case with the black death. Of course it was pretty bad and maybe killed few percents of population causing a lot of additional trouble, but 60-70% numbers sound like fairytales.

In medieval times corona would have wiped out entire villages and towns, so no, the black death today would just be a couple of people in intensive care recovering. You would have to live in a shithole like liberia to die from that

A bigger concern is failure to manage pandemics. The pathogen isn't as much a concern as is the inability to contain it immediately. We live in a much more interconnected world, so our response to potential pandemics should be swift (just look at variance in covid damages between countries).
If by plague you mean the literal plague: we have antibiotics now so it's pretty much a non-issue.

Sure, look at how Covid has spread then realize it isn't very infectious and has a tiny mortality rate.
Something 3 times as infectious and 50 times more lethal would make the Black Plague look like a joke thanks to fast global travel.
