What country? Don't really know how burgers handle it. It's a lot of root memorization and being able to interconnect knowledge from different fields, it's difficult if you have a good memory and are a quick learner, and it's a lot harder if you're not. In general I'd just say you don't have to be the smartest coon on the cotton field to do it, but you have to be willing to pick a lot of cotton for a very long time. Also there's a dedicated med thread so you might want to ask there

Depends on your IQ and country
If you're a mid-iq normie it's gonna be a lot of memorization of stuff that you can just deduce logically/analytically with high IQ
And some countries, like the US, have lots of artificial barriers to limit the number of doctors
