Black holes lose mass naturally via Hawking radiation, just at an incredibly slow rate (all the stars in the universe will have burned out several quadrillion, if not more, times over before the last black hole evaporates). The bigger the black hole - the longer it will take it to evaporate (due to the square-cube law). A sufficiently small black hole will lose mass so fast that it will look like a matter-antimatter explosion; a BH with a mass of 1000 metric tons will emit all of it in 84.1072 seconds. Nothing will be left behind.

If black holes slowly evaporate mass, wouldn't it come to pass that some day in the far distant future they would lose enough mass to no longer be a singularity? Why wouldn't black holes revert to another phase of matter?

Singularity isn't a phase of matter, it's what general relativity predicts to exist inside a black hole, but GR is incomplete. Anything can become a black hole if compressed enough.

There is no coming back from a singularity, removing mass just makes the singularity smaller until you remove all the mass and it vanishes.
