Most likely a human gave the flu to a animal, it mutated and the animal gave it back to another human.

As for why the outbreak reached critical mass before any containment measures could be taken by the rest of the world (especially western nations with critical elections coming), that's where we tip from stupidity and into malice.

China also deleted anyone who was saying it was bad in December. By January welding people into buildings had become a superior save-face strategy to ignoring it. US's attempt at travel bans was half-assed but by then it was futility anyway.
