The most plausible explination I've heard without invoking other universes and more bullshit is Penrose's Conformal cyclic cosmology. Essentially after the heat death everything is uniform so it's equivalent to shrink it down to a singularity (distance has no meaning) and it starts over. Of course like everything there's no proof.

It is not the big crunch. It uses conformal mapping. Instead of having a metric for the universe there is an equivalence class of metrics, and once the universe is in a certain state (that happens to rely on proton decay which hasn't been observed) there is a smooth transition between them.

If you have a thermal state, how can you ask about prior states reasonably? You can do it if you know the scalar potential, but this is a matter of speculation at present.
