1. Erradicate the idea itself from the mind of every living human being
2. Erradicate it from every piece of literature, media, etc
3. Remove all racist terms from all languages
..and you have to do all the above SIMULTANEOUSLY, and INSTANTLY, or the infection will spread again.
Basically, you can't just delete it.
The best you can do is hope that our species evolves, mentally and socially, past the point where it's even a Thing anymore.
Might only take a few hundred years of steady work.
The only other way I can think of to erradicate racism is for all so-called 'races' to be blended to the point that there is no appreciable difference between anyone.
Also not likely to happen.
