I'm not sure if other anons can give an MD POV since I'm just a nurse, but this hospital has done slow codes for cardiac arrests, similar cases to the one I described where its an LTC resident past the point of no return, cancer patients, etc. I'm happy to see that some places in 'Murrica have actually banned the practice.

Not good. Its underfunded as fuck. Nurse to patient ratio is shit. I often find myself taking care of 25 patients on my own depending on the shift. I have to be a nurse and orderly cause we share the 3 orderlies with another unit. I am also forced to work in LTC homes under the management of the hospital, then a covid unit, then back to my IM unit. It's fucking insane and we wonder why the virus is spreading so bad here. We're also forced into overtime even if we just worked a 16 hour day because "well, the charge nurse pulled your name". Sorry for the rant.
And hell, slow codes are not an experience I want to be in again. The docs, nurses, RTs, we all leave feeling like shit. We know the patient is better off dead at this point, but we feel like we let our patient down. Then the family always sends cards to thank us for the care too, of course.

They hire pretty much everyone with vaguely-medical qualifications to do this job in LA.
I've seen nurses, MA's, PA's, 4th-year med students, and other EMT's in this job. Don't know how it is in other states or places, but yeah, anyone with 150 hours of coursework and one or two certifications can probably do this job, and it's only expanding. If you can pick your nose and do undergrad-level biology work it's the easiest money of your life and you can handle a deadly pathogen for 10 hours a day.

That's how it was when I was on the East Coast doing my postbacc shit in spring. I asked about shadowing and volunteer work and they were like, "DON'T TOUCH THE PATIENTS, GO HOME, WASH YOUR HANDS." Everything went fucking online, too.
Accelerated EMT courses only take like 3 weeks here, though, so I just did that and now I get all the patient contact I want. I don't know if I just got lucky or if the bar is way lower now, but yeah, 150 hours of online lectures, above an 80% in the class, passing the NREMT, and getting state-certified for 150 bucks means you're allowed to handle COVID patients in your mid-20's with no previous job experience. If you have a phlebotomy cert it's even better for getting hired.
It's easy as hell, too.
>swab 200 patients a day
>change gloves 200 times a day
>pipette samples into casettes and press "play" on the expensive machine
>run PCR tests
>fill out paperwork
>throw samples in the cooler
>run antigen tests on a sofia machine,
>wear lots of PPE all day long
>sanitize everything with alcohol or bleach
>label stuff

My area has weird specifications as to who can do covid testing. They're hiring dental hygienists, pharmacy technicians, veterinarian technicians, EMS personnel, lifeguards, police officers, security guards, nurses, practical nurses, medical students. Only the nurses fill out all the paper work though.

The actual stuff going on in the lab isn't any more difficult to accomplish than the lab portion of what you'd do in an upper-divisions undergrad science course anyway. Could very well be some kind of healthcare job bubble created by the pandemic or maybe the job is just expanding under high demand. It seems like every other employer is getting tgese tests done for their whole payroll now.

Are you excluding diagnostic studies like angiography, EKG, echo?
You can also look at discoloration of mucosa and extremities, ulcerations, looking for the 6Ps in general.

I personally don't like scrubs because it reveals my man-parts too much. Also, when I get sleepy I usually tend to get an erection as all normal men do. Basically, Just wear something with a belt if you get a snoozerection. To be safe, wear jeans they offer a lot of resistance. If you're a woman, just don't dress like a Mistress of MDs. Also, you may as well dress handsomly because you can afford it and it is expected of you to look like you actually live in a house.

I don't know why Americans love the white coat so much. You just look like a biomed that takes labwork too seriously when labwork is super casual. The best thing about being a doctor in Aus/NZ/UK is that you can wear almost whatever the fuck you want like the preppy fuck you know you are. The best thing is you can flaunt uniform regulations to the nth degree without getting pulled up on it and shit on all other members of staff that have to wear ill-fitting, boxy uniform.

Same, I wouldn't want to wear brogues all day long either. I normally wear docs-style or desert boots since they're comfortable to wear all day round. My chinos/trousers will either sit on top of my boots normally or stack slightly. Of course, given the need to wear scrubs all day in certain places these days, I'll just wear trainers or clogs if I have to.

It's dumb, but it's amazing how much more trust you can engender by looking good, but casually with it. Like you can always tell if it's some short and skinny lad with his shirt buttoned all the way to the top and sleeves rolled like a 12-year-old looking like they're still in school. And they just don't inspire as much confidence.

There are ways to roll shirt sleeves up, and some are more practical than others. Kids tend to have their sleeves rolled up certain ways because they don't know how to do it tidily and roll 4-5 times unevenly because that's how their parents did it for them when they were 5. There are almost no teenagers in our pediatrics wards.
There's always some in ortho, especially in our trauma center, some in surgical wards, maybe sometimes in Gyn. Rarely I see teenagers in pediatrics.

I don't know how it is in America, but lots of other places count anyone under 18 as a paediatric patient, although they tend to vary from case to case when they're 16+. But I've seen a lot of girls with EDs on paeds wards.

my first instinct was to see if there was any literature on their accuracy, presuming that it's simply not detecting the deep sleep, but apparently it (specifically fitbits with the heart rate monitor, in this meta-analysis i found) is fairly accurate, particularly when it comes to REM/deep sleep, compared to polysomnography. Which way are you referring to? I thought you were talking about the army fold where the sleeves are rolled up in a big chunk and kept almost perfectly flat. I used to do that when I was younger because I was in cadets and it was the way I was taught.

One of the top specs IMO. Good mix of operative and conservative stuff, the cases are generally actual medical problems and the spicy time pressure weeds out the neurotics. Always well resourced and staffed, you can just punch out and the next shift takes over from where you left. It tends to attract some low skilled lifesaver monkeys but otherwise the people are great. There is plenty of upwards mobility if you want to move to admin job once you are older.

Went to a different dentist that was open. Took xrays, Had to have a root canal done, nerve was dead and black. I have not experienced any swelling but there is pain related to pressure. But the dentist was aware of the pain and said it was a result of the abscess and said the pain would subside as the abscess is taken care of by my body. He said if swelling developed, or if pain worsened in the next few days, or if I developed any other symptoms of worsening infection eg fever, that I should get in touch and that amoxicillin would be prescribed.

I'm not in anywhere near the kind of pain I was before the root canal. But I'm not opposed to proactively taking antibiotics if it means I'm less likely to get a brain infection or something horrible.

Yeah my biggest worry is spread of infection. Oral infections and abscesses can lead to a bunch of complications so it's best to get it out checked out at a hospital before you develop anything serious.

Believe me, I don't want any kind of spreading infection in my skull either. But Isn't this kind of a slow moving threat, though? So hypothetically, let's say you fracture your T12 after being hit by a drunk driver and while it doesn't affect the spinal cord, three years later you're left with an enduring localised ache and fatigue, a general lack of stamina when standing still and you experience a lot of pain when doing actions that require repetitive motion through that area

I cannot find this answer after hours of googling. Web pages say that swelling is an indication that a root canal was not done properly/sterile environment. But web pages are also saying you should expect swelling within the following 4 days after the root canal. So which is it?

Funny story, I was reading an abdominal film the other day to "assess stool burden" on a patient who, fortunately for me, had a great PCP who took good notes. Apparently, the patient said she shits once a month and she frequently disimpacts herself with a toothbrush. Prior to having this xray, she also said she hadn't shat in weeks. Of course the xray showed literally no stool in her colon. Maybe she was just using that as an excuse to fuck herself in the ass or she was just psychotic.

Regardless, I guess the toothbrush thing might work for you?

idk what to tell him. He's a good friend so I want to give him a thoughtful answer on whether or not he should go for it, but am struggling. the cure exists in the prevention. these things are prevented by lifestyle mainly, barring some rare exceptions.

So many people live their whole lives in relatively poor health, these things wont stop until that changes on a mass scale.

I dunno I feel like US is the one country where you have to go on an exact career tract and be under 30 for M.D. and residency or you're too old to even try. I feel like no one even cares in Europe if you're older or maybe even sees it as better thing than a 20-something wet-nose resident.

If you do your med school in any EU country or britain you usually get the license to practice medicine in any EU country very easily in that you just have to apply for it. So for example if you got some place like Lithuania for your studies you can go for residence in any other EU country.

agreed, this is true. But how realistic/wise is it to start down a path in your early-to-mid 30s that will take min. 7 years to complete before you can really do anything? Not to mention wife, kids, etc. I get the outlier case of someone very passionate about medicine who just didn't have a chance earlier (poverty, health issues, etc.), but this guy has only recently come around to wanting this and has worked 10+ years in another field. Married, no kids yet.

idk, am I just too negative about his prospects? I want to help him make a good choice; perhaps I am too jaded from all the kikery in the profession.

there is no money in UK med. Even in the US, doc wages are declining in real terms. Prestige and money are only in the most competitive specs and even then, the payoff time is long, if that's what matters to you.

Advice: go into medicine for the right reasons. Money matters, but if that's all you want, there are far easier ways to make much more of it than in most of medicine.
