Why are the mentally ill/deficient portrayed (for good reason it seems) as having bad teeth? And not in a crack smoking way, but all the way from childhood

Do people think this? There is obviously an impact.
Anyways you asked specifically about mental health. Bad dental hygiene probably doesn't cause mental illness. More likely that it is a symptom of mental illness along with other hygiene issues.

It’s just a matter of cognition in your example. People who fall for consuming products, especially mass advertised ones like cyberpunk 2077 aren’t exactly what you’d call being self aware of themselves or their environment/surroundings nor their own body.
Basically people who play cyberpunk 2077 don’t know they have teeth and they don’t know what teeth are or what they are used for.
