If someone had an IQ of 1000, as in he is universally true to have an IQ of 1000 compared to an IQ of 100, then likely any type of benign question you ask about genetics, viruses, biology, chemistry, etc, would be answered with "yes". I don't know, because this seems like a topic for EvoDevo biology, which is a topic I don't know much about. However, given my very limited understanding of population genetics and evolutionary biology, it would probably be a lot easier to create two different variants of a virus, one of which will replicate at a much faster rate, and the other which would replicate much slower and lie dormant for awhile before rapidly taking over the other one. This would just involve standard mechanism design, genetic engineering, and evolutionary modeling.

In my dream there was a virus that was super easily transmissible. After sometime it inevitably mutated into something that gave people liver failure.

Yeah, lots of possibilities. eg. in repeat regions you're more likely to get insertion and deletion mutations because the DNA replication proofreading/correction mechanisms don't work well in long repeats.
DNA polymerases have different error rates.
There are millions of DNA modifying proteins that can target specific nucleotide sequences to cut, join, add, remove.

Yes

Probably not since repeat sequences outside genes are effectively ignored. mRNA/protein synthesis enzymes target specific binding sites on DNA so they'd always bind at the start of the gene. A variable AAAAAA repeat in front of the binding site wouldn't make a difference.
You could put a repeat sequence inside a gene but then it relies on mRNA/protein modifications to separate the repeat junk from the deadly part.
Real labs use error prone DNA polymerases for this sort of thing.
