They may be told to not engage in acts that destroy others or harmful to themselves.
Being transgender is delusional just like other identities people swear their lives on. Gender roles are delusional too, even if it's not to the same extent. There's no genetic reason why a male should work 9 to 5 feeding his family, why men have to make the first move in dating, why men are not included in those rape victim group therapy sessions some women have, why men can't cry and bitch as much as women publicy. If you're a man adhering to such expectations, you're delusional. And it can be harmful. But it looks like adhering to this identity, works for most men. What's the issue? The issue then is about how other groups feel, including men who don't want to be like the above.
A lot of women want to look feminine and beautiful, whilst at the same time not being objectified, respected for who they are at work. That's a delusion, tied to their identity. It has nothing to do with genetics. Some end up submitting to this contradictory identity and end up doing mental gymnastics. But if it makes them feel good and if screeching "all men are trash" helps them cope. What's the issue? The issue is if it affects other groups, and women who don't want to live that double life.
Back to transgender issue, it isn't really so delusional when people are given the right hormonal treatment. And with the right technological implementation in the future, there's really nothing stopping someone from looking like they're of any gender they feel comfortable with. It's more delusional today and it does fuck up women. But is it more important for biological women to do sport and rank high than transwomen being allowed?
That's not a question for psychiatry.
