There have been improvements since the first (sometimes disastrous) gene therapies and current techniques are getting quite good but (due to the vectors available) you're still heavily limited by the size of the gene you want to introduce and the area you want to target.
More interesting (imo) are the ex-vivo possibilities. Most of the furthest along covid vaccines with the highest efficacy (94%+) are ones that were produced using modern biotech techniques.

Shit's still going to happen anyway, only a matter of time. Personally I would be quite keen on genetically modifying ONLY my gametes to ensure I have offspring without the defective genes I carry (myopia and the atopic triad: asthma, eczema, allergies).
