Most everyday situations where I’m actively focusing on/doing something is, generally, “wordless”.
However, if I read/write, or start to have an intellectual thought process develops where I’m reasoning, it’s usually in the form of a steam of consciousness, first person “thought-tunnel”.
There’s more in between states, such as when I’m doing a more sequential task, like math problems, where I’m actively reasoning in my head about how to proceed, while most of the actual calculations are done in a more “imaginary” visual space (if this internal narration is one of the auditory).
There’s also times where my mind is like yours, and silent. For instance, as I write this, as I am beginning to typing each letter, I hear the word in my head being brought forth to the nucleus of my attention.
But when I switched apps to change a song and take a hit of my bong after finishing the last paragraph, I simply acted and with no internal commentary.
