What is mental illness?

While we don't understand how exactly the brain functions, or what roles individual parts of the brain play in a persons behavior is their are sort of "fuzzy" picture of mental illnesses?

Can science at least begin to guess why people with mental problems exhibit the behavior that they do? Can for example, we explain why higher IQ people like Terry understand that they are unwell—while people like your average IQ california-crazy have no idea that they are ill?

From our current understanding what is the perspective on delusional disorders? And what about people who are neuro-divergent; like psychopaths and sociopaths? What about people with ADD or ADHD? Are all of these related? Or are they all completely different and stem from developmental issues of different parts of the brain? Is there any correlation between mentally ill people and their brain structures? Or are the issues only connected with the general idea of neuro-divergence?

Genuinely curious, reading wiki on it but I feel like /sci would have a lot of anecdotes about an interesting topic like this.
