Anesthesia for surgery

If we know there are cases of individuals "waking up" during surgery. As well as some cases of people forgoing anesthesia to prove their mind of matter capabilities.

What are the chances, that some people have died from mental shock? And not so much a "dangerous" surgery.

If that is the case why is it wrong to give people a "trial" run with anesthesia.. and then them being pricked or lightly shocked... to see if they notice. Because if they do, then the patient and the doctors both know the risk. That the anesthesia will keep the patient from moving.. but they will both know the patient is mostly awake during surgery?
