For the more complex stuff i'm usually better off just focusing 100% of my attention on following the lecture then writing my own notes afterwards using the book / any insights i gained. Basically using the Feynman approach.
For more slow paced lectures, where the lecturer writes good, comprehensive notes on the blackboard as he goes i'll basically just copy those with my own additions.
