a) Build on top
You can expand it as far as fictional structural integrity let you, unless you build and link new engines, not all engine need to be as powerful, top don't need to be same height.

b) Around it (in case it need a shaft across the entire ship)
If it's not 1G and require rotation as well, you can end up with interesting shape, "ring/flat platform" that are 45° on the side and one side let you see the next ring.
Remember than not EVERY place need to be 1G, so if some place can be less or 0G someone will want one.
