How many publications are you supposed to have by the end of your bachelors, masters and PhD?
I've finished my bachelors with one.

depends entirely on the field
my bachelors I worked in a lab whose experiments typically took 5 years to get from start to finish. I didn't get any papers as a result, but now I'm in a top PhD program because the industry understands how different fields lead to different amounts of publications

if you're in any bio/chem field there's no excuse to not have 3+ papers though.

I'm in my second year of a math + CS degree. I got middling grades first year, but now I'm properly Adderalled up and ready to sort my life out. But is it too late for me to get meme internships and papers for grad school?

2nd year? you're fine, that's when a lot of people start. and in cs you can work quickly since you're not relying on being in a lab.

Finished my PhD (cell bio) with 2 papers, like 7 middle author.
First month at my new job (machine learning + drug discovery stuff) and I'm already middle author on 2 papers and writing up my first.
Different fields, different worlds.

if you don't have any in-person contacts with professors you'd want to work with, then just email different ones you're interested in working with.
be sure to mention your background and interests as they pertain to the research that the lab is doing

In regards to first author publications: I didn't have any after my bachelor's, 1 after my masters, 2 total after my PhD, 4 after my postdoc, and now 6. I currently have 2 under review - submitted one today, actually.

So we take undergrads in our lab (I'm a grad student) and having your name on a paper is still rare-ish, but we do have a lot of undergrads now trying to see if they'll have a chance at throwing their name on a paper or not before agreeing to join the lab instead of actually caring about the research which is annoying

Sample size of 6 are usually for case studies. Case studies can play their part aslong as you can treat it for what it is. If you are studying a rare disease that only 1/million people have you’d expect something like this.
