It's most commonly used to indicate:
1. That a person is retarded or stupid.
2. That a person is a genius or has an intellectual/mental ability which is not attained by "normies"

4. It may indicate that a person is perceived as having poor social skills/being rude or arguable.
5. It may mean being obsessed of or being hyperfocused on something. Words like sperg and weaponized autism emerged originally from 5.
6. It may mean a bunch of other things like: being too anxious, a lack of empathy, a perceived lack of individuality, too much idiosyncratic individuality, having lost connection to the real word where it substitutes schizo.
Like any other psychiatric condition, its use is mainly negative although unlike most other insults, autism and autistic may have positive connotations. I have also noticed that people also use autistic as a tool to describe themselves rather than others..
