In the military, you can apply for the Officer's course and climb the ranks of commanders. Vets can even take the role of trainers or be specialized. SWAT and Sniper course require 2 years of experience for example.
Don't know about Medicine but pretty sure they too have Heads and Supervisors
Astronauts have fields like Commander, Payload, Mission Specialist, PIlot, etc.
