Death or health failure seems more related to BMI. Once you fall under 15, you're in trouble, and that's what happened in the shorter documented cases. Someone starting from a morbidly obese state will take much longer to reach that BMI level, so there's potential there for a much longer fast. Whether or not Angus Barbieri's case can be replicated, who knows, starvation isn't something most people choose to inflict upon themselves (and those who do seem to limit it to days, with a few exceeding a month). We can't say for sure if Angus was nibbling on something since he was an outpatient, but to drop that amount of weight in that time frame would have required a 2,500+ calorie deficit.
