Telescopic pictures exist mate. Also, that's our neighboring galaxy Andromeda, not the Milky Way. However, refined artistic renditions also exist, and even 3D CG representations from composite image data exist.

When I point that out to "i fukin love science"-type people they come up with so and so excuses as to why they believe that "scientists know what the milky way looks like" (e.g: they know what it looks like because they can look at other galaxies), but the official answer is that they don't know what the milky way precisely because it's not possible to take a picture of it from the outside, and the "spiral" model is based on an assumption. We could very well be in a shapeless cloud instead.
