Painkiller delivery method

Is there a more effective painkiller than the morphine with all the risk of respiratory depression? Can't the pain signaling be blocked locally?
The pain start pheripherically.. can't be blocked there instead of having the morphne pass the blood brain barrier?
Isn't there another molecol that binds to opiod receptors pheripherically but not in the SNC?
