To acquire a position (academia), I compiled a list of relevant (and interesting) professors at each university that I'd be willing to relocate to and emailed them directly, told them that I have had some time off but like their work and think I could bring my skills to their team. Some did not respond, some responded very enthusiastically, and I landed a very prestigious position doing this. Just be personable.
If you are looking for an industry job, perhaps contact a professor who you had good rapport with and tell them you are rekindling your interest in the field and would like them to act as a referee if possible.
You'd be surprised at what you can achieve by just talking to these people - they are real humans, and the perceived culture of 'falling off the train' is largely untrue and built around fear.
